A crush injury, and the gap between policy and the shop floor
The Health and Safety Executive reported on 18 August 2026 that a waste and recycling company had been fined £400,000 after a woman was crushed at its site. That is the headline. For anyone running SHEQ in the sector, the more useful question is how a person and a moving machine came to occupy the same space at all.
If you run SHEQ for a waste, recycling or heavy-materials operation, the pattern is familiar. The recurring phrase in cases like this is 'custom and practice'. It describes the gap between the safe system of work in your management file and the routes people actually walk at hour eleven of a shift. That gap is where these incidents happen, and closing it is the real subject of this article.
The law already tells you what to do
The regulatory expectation here is settled, not contested. Section 2(1) of the Health and Safety at Work etc. Act 1974 requires employers to ensure, so far as is reasonably practicable, the health and safety of employees. The Provision and Use of Work Equipment Regulations 1998 (PUWER), and its ACOP L22, require work equipment such as a mobile shredder to be suitable, maintained and used with appropriate safeguards, with Regulation 11 addressing dangerous parts and crushing risk. The Workplace (Health, Safety and Welfare) Regulations 1992 require traffic routes that keep pedestrians and vehicles apart where reasonably practicable.
HSE guidance is equally specific. INDG199 and the workplace transport pages name separate pedestrian and vehicle routes as the most effective control, with protective barriers and guardrails at entrances, corners and crossings where full separation is not achievable. WISH WASTE-09 (V2) sets a preference order for physical barriers on recycling sites. On the machinery side, the Supply of Machinery (Safety) Regulations require new plant to be designed and guarded so that crushing and shearing points are prevented or protected, and HSE's guidance on the law for the design and supply of products sets out those duties.
So the design answer is well understood. The problem these cases expose is not that nobody knew what to specify. It is that a static control, once installed, cannot notice when it is being walked around.
Physical barriers work, until someone steps past them
WISH WASTE-09 lays out the hierarchy every specifier already knows: solid concrete or permanent construction first, then double Armco or vehicle-profile designs, single Armco, permanent railings and fencing, removable barriers, and raised kerbs, usually in combination at pedestrian exits, crossings and tipping areas. The market splits cleanly on materials.

All of this is sound engineering and none of it is optional. But every one of these controls shares a limitation. A barrier defines where people should not go. It does not know when a shredder is moving, it does not know a pedestrian has crossed the line, and it cannot stop the machine. At a gate post, at a tipping face, at the moment a reversing vehicle and a person occupy the same metre, the barrier has already done everything it can do. SAIFI is designed for exactly that moment.
Active safety: catching the metre the barrier cannot
Most cameras on a waste site are passive. They record footage that gets reviewed after an incident, which is useful for an investigation and useless for prevention. SAIFI Edge inverts that. It is an AI computer-vision system that watches the vehicle-pedestrian interface in real time, recognises a conflict as it forms, and triggers a physical response: closing a gate, dropping a barrier, projecting a warning onto the ground, sounding an alarm, or cutting access. It does not replace your banksmen, your yellow lines or your segregation routes. It is the last line that acts when a person has already crossed one.
Think about the geometry of a crush point: a person and a moving machine converging on a fixed gate post. A SAIFI-linked maglock gate post holds or closes when a pedestrian is detected in the conflict zone, removing the crush point rather than documenting it afterwards. That is the difference between an evidence file and an intervention.

This is a pattern, not a one-off
This £400,000 fine is one case in a run of HSE prosecutions with the same root causes: missing segregation, weak monitoring of controls, and systems that existed on paper but not in practice.
- •Biffa: fined almost £2.5 million in October 2025 after a worker was crushed by a skip wagon.
- •A builders' merchant: fined £2.2 million in March 2026 after a worker was killed in a conveyor crush.
- •A Worcester waste company: fined in October 2025 after a worker suffered life-changing crush injuries.
- •A waste management company: prosecuted in April 2026 after a worker was crushed by an excavator.
The recurring themes across all of them are inadequate risk assessments, lack of segregation, and failure to monitor whether controls are actually being followed. Barriers and procedures address the first two. Real-time detection and intervention address the third, which is the one that keeps failing.
For the board paper
- ✓Map every point where pedestrians and mobile plant can converge: gates, crossings, tipping faces, weighbridges.
- ✓Confirm your barrier hierarchy against WISH WASTE-09 and record where full separation is not reasonably practicable.
- ✓Identify residual crush points that survive the barrier design; these are your active-monitoring candidates.
- ✓Ask whether your existing cameras prevent incidents or only record them.
- ✓Cost the intervention against the current run of six-figure and seven-figure fines, plus insurance and downtime.
The regulator's expectation is not going to soften. Enforcement in this sector is active and the fines are scaling. The defensible position in front of a board or an inspector is a layered one: designed segregation, physical barriers where they belong, and active detection that acts at the crush point when a human inevitably strays past the line.
