Vehicle and Pedestrian Separation in US Warehouses: Where Barriers Stop and Detection Starts

A run of recent US cases, from a $288K OSHA penalty to a $5.5M forklift verdict, shows that physical separation alone leaves a gap: barriers and paint don't react when a pedestrian and a forklift arrive at the same point at the same second. This article maps the separation hierarchy US specifiers already use, then shows where active detection catches what the layout can't.

The separation is designed. The collision still happens.

You already run an internal traffic plan. Yellow guardrail lines the main aisles, bollards protect the dock faces and the charging bays, and the layout pushes forklifts and people down separate routes wherever the building allows it. That is the correct foundation, and it matches what US specifiers and regulators expect. Yet the incidents that reach the news, and the settlement columns, keep happening inside sites that had barriers installed. The gap is not the plan. It is the single moment when a pedestrian steps past the end of a guardrail and a reversing forklift arrives at the same square metre of floor. A bollard cannot see that. A painted line cannot act on it.

What the recent US caseload actually shows

Look at the pattern across the past two years rather than any single event. In December 2024, OSHA proposed $288,000 in penalties against Mitchell Industrial Tire following a forklift-related fatality. In April 2026, a Wisconsin jury returned a $5.5 million verdict against Menards over a forklift injury at a distribution centre. In May 2026, an Oregon lawsuit sought up to $35 million after a teenage worker was crushed by a wheel loader, an incident that also drew a serious OSHA citation of $31,632. In September 2025, a worker in Baldwin, New York was killed after being crushed between a forklift and a tractor trailer. Different sites, different equipment, the same failure mode: a person and a machine occupying the same space at the same time.

OSHA does not mandate bollards. It requires marked, clear aisles with safe clearances under 29 CFR 1910.176(a) and 1910.22, and it leans on the General Duty Clause where a recognised hazard, such as a documented near-miss or prior damage, goes uncontrolled. Warehouse citations for unmarked pedestrian zones and blocked aisles carry escalating penalties where findings are repeated or wilful. The regulatory direction of travel is toward physical control, not signage. That is the same conclusion HSE reached in the UK under Regulation 17 of the Workplace (Health, Safety and Welfare) Regulations 1992 and its HSG136 guidance, which prioritise complete physical segregation wherever reasonably practicable.

The hierarchy US specifiers use, and its ceiling

Firms like Damotech, Axelent USA and OPSdesign converge on a layered model, and it is a sound one. Eliminate the intersection first through layout, then separate physically, and only then fall back on behavioural controls.

  • Design out the conflict: one-way vehicle routes, dedicated pedestrian zones, elevated walkways and geofencing or RTLS to remove intersections entirely in new distribution centres.
  • Protect the fixed points: bollards at loading docks, crossings, charging stations and building columns, matched to the impact energy of the vehicles that use the area.
  • Run continuous separation: polymer or steel guardrail along walkways, with polymer favoured for energy absorption and reduced floor damage, steel for raw strength.
  • Anchor for the site: surface-mounted barriers for reconfigurable warehouses, embedded for permanence, at practical spacing with ADA-compliant egress clearances.
  • Support with visibility: high-visibility finishes, mirrors, signage and high-vis clothing as reinforcement, never as the primary control.

Every layer in that stack is static. A guardrail installed on Monday exerts exactly the same force on Friday whether the aisle is empty or a banksman just lost sight of a pedestrian behind a stacked pallet. Physical separation sets the boundaries of where a collision can happen. It does nothing about when. On the hundredth hour of a shift, when a driver takes the short cut everyone takes and a pedestrian crosses where they always cross, the layout holds but the moment slips through.

Where detect-and-act fits alongside the barriers

SAIFI Edge is a computer-vision layer that sits on top of the separation you already have. It watches the specific conflict points your risk assessment already flagged, the dock approaches, the pinch points where a walkway meets a vehicle route, the ends of guardrail runs, and it recognises a vehicle-pedestrian conflict as it forms. It is not a replacement for anything on your site. The banksman keeps directing, the yellow lines stay down, the mirrors stay up, the training programme continues. Physical separation decides where a collision can happen. Detection decides whether it happens at all.

The argument to take to your board

The cases above are the ROI model, unfortunately written by other operators. A $288,000 penalty, a $5.5 million verdict, a lawsuit seeking $35 million: each began as a single unmanaged moment at a point that likely already had some physical control nearby. Add active detection at your two or three worst conflict points and you are buying down the tail risk that barriers alone leave exposed, while strengthening the audit position you present to OSHA or, for UK and EU sites, to HSE under HSG136. The install is targeted, not a rebuild. It reads your existing layout and adds the reaction layer where the geometry and the traffic data say the risk concentrates. A site survey is the sensible first step: it tells you where the gap is before you spend against it.
